LAVENDER FOAL SYNDROME

To assist with the ordering process, below are instructions for submitting samples:
- Download and fill out the sample submission form: http://ahdc.vet.cornell.edu/pdf/lavendarfoalform.pdf
- Download the sample sheet for attaching the hairs: http://ahdc.vet.cornell.edu/pdf/lavendarfoalhair.pdf
- Pull 50 mane hairs, trim to 4" in length (DO NOT TRIM OFF THE ROOTS) and tape them to the sample sheet (#2 above), fold up the sample sheet around the hairs, and put it in an envelope with the horse’s name and registration number included on the envelope; then staple the envelope to the submission form (#1 above) at the place noted on that form.
- YOU MUST PAY BY CREDIT CARD unless your vet has an account at the Cornell AHDC and is submitting the samples for you. To pay yourself, download the credit card authorization form and fill it out: http://ahdc.vet.cornell.edu/pdf/ccPaymentAuthorization.pdf
The cost is $47 for each sample ($45 for the test, plus $2 accession fee). - Send all three of the above forms to Cornell at the appropriate address listed on the sample submission form.
Note: the sample submission form has two different addresses, depending on how the sample is being sent (regular mail or UPS/FedEx). Be sure to use the correct address.
Some Important Notes:
- You do NOT need to have a vet involved.
- You must fill out a separate sample submission form AND sample sheet for each horse.
- You may submit multiple samples together in one big envelope, but be sure to staple each sample envelope SECURELY to the correct matching submission form.

VOLUNTARY REPORTING OF TEST RESULTS: If you wish to report the results via F.O.A.L. for public disclosure (they also maintain a list of results for SCID testing), use the F.O.A.L. LFS Test Report Form which can be accessed at: http://foal.org/Test-Kits---Forms.html and select Lavender Foal Syndrome Cornell U. Test Report Form.
